2. A description of the proposed content of the masters degree program, length of program, and location.

3. A narrative of how the proposed masters degree program will a) improve your personal growth and performance as a MSUE Professional and b) substantially contribute to the effectiveness of MSUE programs.

4. An itemization of estimated costs, other scholarships or sources of funds to be used, and amount requested from this fund.

Administrative Approval:

Approval is needed from your immediate administrative supervisor via email (preferred) to Kathy Foerster or a hard copy may be sent to the Kathy Foerster, Room 11 Agriculture Hall, Michigan State University, East Lansing, MI 48824-1039 identifying the following:

The supervisor understands that the applicant is an MSUE staff member with at least one year of employment at 50% appointment with MSU Extension, is not in the last year of employment with MSU Extension, and has approval to submit this application.
