Click here for a printable form: Sample Submission Form4.pdf to send in with your soil sample, or obtain one from your county MSUE office. Each form has space to describe several soil samples. The sample form contains instructions for filling it out. The standard soil test is adequate for most home gardening situations, but you may wish to request additional options such as Organic Matter or Particle Size. Those options and tests for specific micronutrients such as zinc are extra cost options. Be sure to specify any extra cost options for each sample on the form, using the last column and the Comments section, and include payment for the options with the sample. (See prices.) On the back of the form is a list like the one below of crop categories for home, urban and special crops. You should specify, by number, for which crop category you wish the lab to calculate fertilizer recommendations. If not sure, specify your first and second choices.
