Michigan 4-H Youth Participation Fee Information

Beginning with the 2006-7 program year, the Michigan State University Extension 4-H Youth Development program has asked youth participants to pay an annual $10 participation fee to support the development, implementation and maintenance of new or updated 4-H programs.

The County Form is for counties to use when sending the state portion of the fee to the State 4-H Office (page 1) and to report how they are using fee dollars (page 2).

Grant Reporting Forms

  • Initial Project Report (due 60 days after project completion) is for grantees to report the project's outputs.
  • Final Project Report (due 1 year after project completion) is for grantees to report on the new program's impacts and outcomes.
